Service delivery details
The SLEAT Behaviour Consulting agrees to provide the participant with support as stated in the NDIS plan or as is reasonable and necessary based on the participant's needs and funding availability. Services are provided based on evidence-based practices in the fields of Applied Behaviour Analysis (ABA) / Positive Behaviour Support (PBS) and may include, but are not limited to:

Published research and our clinical experience demonstrate that the best outcomes are achieved when parents, guardians and caregivers play an active role in carrying over therapy activities outside of sessions. To help achieve the best outcomes of therapy, all of our service delivery options include family coaching and education.
The cost for the services is set out further in your respective service agreement. The supports will be delivered as per the schedule of supports recommended where there are multiple services being delivered.
The SLEAT Behaviour Consulting only provides services to private clients, NDIS self-managed and plan-managed participants. The SLEAT Behaviour Consulting will invoice you or your nominated plan manager for services delivered, upon completion of appointments or support services. For agency-managed participants, The SLEAT Behaviour Consulting is subcontracting with To the Moon and Back Behaviour Services to provide requested services. To proceed, a service request should be made under To the Moon and Back Behaviour Services and a separate service agreement will have to be signed. In this case, To the Moon and Back Behaviour Services is responsible to make claims for services provided directly from the NDIA.
By ensuring that all charges are transparent and communicated proactively, where non-face-to-face tasks are expected to require significant time (more than 15 minutes), the practitioner will notify the participant or their representative beforehand, explaining the nature and purpose of the task and obtaining verbal or written consent where necessary. For ongoing non-face-to-face services (e.g., regular liaison with other professionals), participants or their representatives will be updated periodically about the time and cost associated with these activities. Please refer to your service agreement for further details.
Payment terms
The SLEAT Behaviour Consulting accepts clients with private funding, self-managed NDIS funding, and plan-managed NDIS funding. However, we are unable to accept NDIA-managed (agency-managed) funding due to registration requirements. If your funds are NDIA-managed, any Supports to you will be out of pocket as we are not a registered NDIS provider. If you'd like to access services using your NDIA-managed funding, a service request should be made through To the Moon and Back Behaviour Services and a separate service agreement has to be signed.
As an NDIS participant, you are responsible for monitoring and managing your NDIS funding to ensure it aligns with the services you wish to receive. While The Sleat Behaviour Consulting will provide you with the necessary invoices and details of the services provided, it is important that you regularly review your funding balance to ensure that sufficient funds are available for continued service delivery.
We kindly encourage participants to keep track of their funding and promptly inform us if there are any concerns or if additional support is needed with funding management. This helps to ensure that there are no disruptions to your therapy sessions and that we can continue to support you effectively in achieving your goals.
If you are a private or NDIS self-managed participant:

The Sleat Behaviour Consulting will provide you with an invoice after services are delivered. You are required to pay the invoice within 7 days of receipt. If payment is not received within 7 days, an overdue notice will be sent to the your nominated email address. If payment remains outstanding for 14 days, therapy services will be placed on hold for a maximum of 2 weeks until the outstanding invoice is paid. Therapy services will resume once payment is received, provided that a therapy slot is still available after the 2-week hold period.
If you are a NDIS plan-managed participant:

You will nominate a registered plan management provider to manage the funding for NDIS supports provided under this Service Agreement. After services have been delivered, The Sleat Behaviour Consulting will claim payment for those supports directly from the nominated Plan Manager.
