Although we are not an NDIS-registered service provider, we comply with all applicable privacy and confidentiality obligations as an NDIS-registered Behaviour Support Practitioner.

    • Information we receive from other stakeholders, including healthcare providers, allied health professionals or other agencies (this may include but not limited to your NDIS local area coordinator/planner, schools/preschools/day care/work places) with your consent.

  • Full name, email address, street address and phone number

  • Demographic information (e.g., date of birth, age, gender)

  • Health information relevant to the services we provide, such as your disability type, medical history, progress notes, current therapy goals, and treatment plans

  • NDIS participant information, including your NDIS plan details and funding (if applicable)

 

We collect this information to provide tailored services and ensure that we meet your individual needs.

How we use your information

We use the personal information we collect for the following purposes:

  • To provide you with the services you request, including clinical supervision, behaviour support services, and relevant consultation services

  • To communicate with you about your care, progress, and any changes to services or appointments

  • To assess and improve the quality of our services, ensuring compliance with NDIS best practice standards

  • To manage billing, payments, and administrative processes

  • To comply with legal and regulatory obligations, including those outlined by the NDIS Quality and Safeguards Commission and the Privacy Act

  • To advise you if our Terms and Conditions or Privacy Policy have been updated or amended

  • To update you of changes to service conditions

  • To keep you informed about new resources, or other relevant matters

  • To collaborate with your stakeholders, with your consent, to provide the services you request

We will only use your information for these purposes and will not use or disclose your personal information for any other purpose without your consent, unless required by law. Your personal information will not be shared, sold, rented or disclosed other than as described in this Privacy Policy. We will immediately remove your personal information from its database if requested by you.

The SLEAT Behaviour Consulting uses the following electronic, wed-based services to store and use your data. They may include:

    • Used for session notes, progress report writing, behaviour support plan writing, document storage, and communication logs. Data is hosted in Australia and complies with Australian data protection laws.

  • Splose https://splose.com/ 

    • Used for scheduling, clinican team communication, collection and storage of personal and health information, invoicing, service agreements.

  • VB-MAPP https://www.vbmappapp.com 

    • Used for progress monitoring.

  • EFL https://eflapp.com/

    • Used for progress monitoring.​

  • Xero Accounting https://login.xero.com 

    • Used for reconciling accounting transitions.

  • Microsoft Exchange https://products.office.com 

    • Used for sending and receiving emails via the thesleat.com domain.

  • Zoom and Zoom AI Companion https://zoom.us/

    • Used for videoconferencing/telehealth sessions and meetings. The Zoom AI Companion may provide features such as meeting summaries and note-taking. These tools are used only with your explicit consent. Any AI-generated content is managed in accordance with our privacy and confidentiality obligations.

  • Microsoft Teams https://products.office.com

    • Used for videoconferencing/telehealth sessions and meetings.

 

Where electronic data is transmitted to internationally based servers, The SLEAT Behaviour Consulting has taken steps to ensure that data is maintained under security and privacy principles that provide a level of protection at least as secure as those required by the Privacy Act 1988. Specifically, the Privacy Act imposes specific rules regarding the transfer or disclosure of personal information to overseas entities.
